To determine if you qualify, please bring the following items to your appointment -

Photo Identification for all adults
Proof of Age and Social Security Number for all household members
4-6 of your most recent paystubs
Proof of all forms of income like TANF, Social Security, Disability or Child Support (Examples of acceptable documents include award letters and printouts directly from the websites for these agencies dated within the last month)
In some instances, bank verifications will be required
